We hereby make an application for credit to Pacific Spice Co., Inc. This information in given is strict confidence for the sole purpose of establishing credit with Pacific Spice Co., Inc. Authorization is hereby given to make inquiry of all trade and financial sources, which are deemed to be necessary by Pacific Spice Co. Inc. to properly evaluate this application. If credit is granted, we agree to pay all bills within the stated terms of sale on the invoice. I also understand that Pacific Spice Co., Inc. reserves the right to assess a service charge of 11⁄2 % per month or 18 % per annum on all past due invoices. Should legal action be taken to secure payment for product received, we will be liable for all expenses, including, but not limited to reasonable attorney’s fees. This agreement shall be deemed to be made under the laws of the State of California and shall for all purposes be construed and enforced in accordance with California Law and any and all disputes hereunder shall be resolved in the courts of Los Angeles County, California.

The undersigned individual who is either a principal of the credit application or a sole proprietorship of the credit applicant, recognizing that his or her individual credit history may be a factor in the evaluation of the credit history of the applicant, hereby consents to and authorizes the use of a consumer credit report on the undersigned by the above-named business credit grantor, from time to time as may be needed.
